本活動屬於高風險戶外活動，參加者須自行準備合適且符合安全需求之裝備。
領隊已向您說明登山與徒步活動可能涉及之風險，並提醒您於參加活動前，應審慎評估自身健康狀況、體能及能力，並自行決定是否適合參與本次活動。
參加者應誠實告知自身健康狀況，包括但不限於心血管疾病、高山症病史、氣喘、重大疾病或近期受傷情況。如因隱瞞而導致事故或影響團隊安全，相關責任由參加者自行承擔。
參加者須自行負責購買足夠且有效之旅遊與意外保險，以保障自身權益。
參加者需自行安排並準時抵達指定集合地點。如因個人因素，包括但不限於航班延誤、交通問題、簽證因素、個人健康狀況或其他私人原因，導致無法準時集合、缺席活動或中途無法參與，本公司概不負責，且相關費用恕不退還。
於活動期間，參加者必須遵守領隊、嚮導及工作人員之安全指示，不得從事任何可能危及自身或他人安全之行為。
若參加者有危險行為、不服從安全指示、影響團隊秩序，或經領隊判定不適合繼續活動，本公司有權終止其行程參與資格，且不予退款。
若參加者於活動途中自行退出，或因身體不適、受傷等原因需提前下撤、就醫或安排救援，所有相關交通、住宿、醫療、救援及其他衍生費用，皆由參加者自行承擔。
因天氣、天災、道路狀況、交通延誤、政府限制或其他不可抗力因素，領隊與嚮導有權依現場狀況調整、更改或取消部分行程內容，以確保團隊安全。參加者須配合安排，不得因此要求賠償或退款。
請妥善保管個人物品，如有遺失、損壞或遭竊，本公司及工作人員概不負責。
活動期間所拍攝之照片與影片，可能用於本公司之網站、社群媒體及宣傳用途。如不同意公開使用，請提前告知。
參加者應理解並同意，登山及徒步活動具有不可預測之風險，包括但不限於受傷、意外、疾病、失聯或死亡等情況。對於因參與本活動而導致之任何損失、傷害或事故，本公司、領隊、嚮導及相關合作單位概不承擔責任。
Acknowledgement & Consent Form
This activity involves inherent risks associated with outdoor trekking and hiking. Participants are responsible for preparing suitable equipment and gear necessary for their own safety.
The tour leader has explained the potential risks involved in trekking activities and has reminded you to carefully assess your own physical condition, health, and abilities before participating. You are solely responsible for determining whether you are fit to join the activity.
Participants must truthfully disclose any relevant medical or physical conditions, including but not limited to cardiovascular disease, history of altitude sickness, asthma, major illnesses, or recent injuries. Any consequences arising from failure to disclose such information shall be the sole responsibility of the participant.
Participants are responsible for obtaining adequate travel and personal accident insurance coverage at their own expense.
Participants are responsible for arranging their own transportation and arriving at the designated meeting point on time. The company shall not be held responsible for any failure to join the activity due to personal reasons, including but not limited to flight delays, transportation issues, visa matters, personal health conditions, or other private circumstances. No refunds will be provided for missed participation resulting from such reasons.
During the activity, all participants must strictly follow the instructions and safety guidance provided by the tour leader, guides, and staff. Any behavior that may endanger yourself or others is strictly prohibited.
The company reserves the right to terminate a participant’s involvement in the activity without refund if the participant engages in dangerous behavior, fails to follow safety instructions, disrupts the group, or is deemed unfit to continue by the tour leader.
If a participant withdraws from the activity midway, or requires evacuation, medical treatment, rescue services, or early departure due to illness, injury, or personal reasons, all related transportation, accommodation, medical, rescue, and additional expenses shall be borne entirely by the participant.
Due to weather conditions, natural disasters, road conditions, transportation delays, government restrictions, or other force majeure circumstances, the tour leader and guides reserve the right to modify, cancel, or adjust the itinerary for safety reasons. Participants must cooperate with such arrangements and shall not claim compensation or refunds as a result.
Participants are responsible for safeguarding their personal belongings. The company and staff shall not be liable for any loss, theft, or damage of personal property.
Photos and videos taken during the activity may be used for the company’s website, social media, or promotional purposes. Participants who do not consent to such use must inform the company in advance.
Participants acknowledge and accept that trekking and hiking activities involve unpredictable risks, including but not limited to injury, accident, illness, disappearance, or loss of life. The company, tour leader, guides, and related parties shall not be held liable for any loss, injury, accident, or damage arising from participation in the activity.
本人已仔細閱讀並充分理解上述聲明內容，並確認已評估自身健康狀況與身體能力。我願意自行承擔因參與本次登山／徒步活動所可能產生之一切風險與後果，並同意遵守活動期間所有安全規範及指示，同時自行負責購買足夠之保險保障。
I have carefully read and fully understood the above statement. I confirm that I have assessed my own physical condition and fitness level, and I voluntarily accept all risks and consequences that may arise from participating in this trekking/hiking activity. I agree to comply with all safety rules and instructions during the activity and accept full responsibility for obtaining adequate insurance coverage for myself.
